Member Reference * Provide the full name of a current CHA member who can vouch for your application. The membership committee will verify their membership status.

I have read and agree to the CHA Constitutions and Bylaws. I understand that membership requires a $75/month automatic withdrawal.

The Association will use your information solely for membership administration, communication, and related Association activities.
